A copy of the orthodontia contract (also referred to as a "Truth in Lending Agreement") should clearly show the following:

• Who the treatment is for
• Total cost of the treatment
• Start and end date of treatment
• Estimated length of treatment
• Initial payment/down payment amount
• Amount covered by insurance, and
• Installment payment amounts and frequency

If orthodontia treatment is being financed through a third party lender, please provide a copy of the agreement with the lender in addition to the contract with the orthodontia provider.
